Cancer screening should have better pathways than being included in medical insurance

Recently, the National Medical Insurance Administration responded to 11 proposals from the first session of the 14th National People's Congress and the first session of the 14th National Committee of the Chinese People's Political Consultative Conference. Among them, in response to the suggestion to include tumor screening in medical insurance reimbursement, the National Medical Insurance Administration replied that the conditions for including cancer screening in medical insurance are not yet mature, and non therapeutic items such as tumor screening cannot be included in medical insurance payment scope temporarily. There has been a lot of discussion in society about whether cancer screening should be included in medical insurance reimbursement. In fact, a more fundamental question than this is whether physical examinations should be included in medical insurance reimbursement. Because cancer screening is usually a physical examination, it is not a necessary diagnostic and treatment item and is a prerequisite for discussing this topic. If a patient is highly suspected of having a certain type of cancer and undergoes some tests based on indications for diagnosis, it is a necessary process for reasonable diagnosis and treatment, and medical insurance can reimburse them. Physical examination is aimed at healthy individuals, and every healthy person can participate. If physical examinations are included in medical insurance, it will be difficult to control the number of physical examinations guaranteed by the medical insurance fund, and the payment pressure borne by the medical insurance fund will suddenly increase. Therefore, society generally believes that physical examinations should not be included in medical insurance reimbursement, at least the conditions are not yet mature. Moreover, compared to regular physical examinations, cancer screening involves many expensive examination items, and it is even more cautious to include cancer screening in medical insurance. In fact, cancer screening has a better solution than being included in medical insurance. In basic public health, there are projects for preventing and controlling cancer. For example, in 2009, China included the examination of rural women's cervical cancer and breast cancer ("two cancers") in major public health services; Starting from 2019, "two cancer" examinations have been included in the national basic public health service projects. At present, the "two cancer" free screening project has been carried out for many years and has achieved fruitful results. Especially, measures such as free HPV vaccination targeting women around the age of 14 in many regions will significantly accelerate the final elimination of cervical cancer. The practices of basic public health in preventing and controlling multiple cancers in women are worth learning from. If financial resources permit, especially in areas with relatively good economic conditions, priority can be given to including some common cancers that pose great health risks in the key content of basic public health projects. By providing free physical examination and early intervention to key populations, the incidence rate of related cancers and their health hazards can be significantly reduced. In addition, the role that commercial insurance can play in cancer screening cannot be underestimated. We can consider launching more high-quality insurance specifically for cancer, providing financial support for cancer screening and subsequent treatment through mutual assistance. I believe that as long as the relevant insurance types are designed scientifically and reasonably enough, a win-win situation between policyholders and insurance companies can be achieved, and another good way for cancer screening can be found. Popularizing cancer screening is of great benefit to reducing the incidence rate of cancer and reducing the economic burden caused by cancer. All suggestions that are conducive to popularizing cancer screening are worth listening carefully. At present, the conditions for including cancer screening in medical insurance are not mature, but the possibility of mature conditions in the future should not be ruled out.
