In order to safeguard the health rights and interests of patients, ensure their safety, and further improve the level of patient safety management in medical institutions, the National Health Commission recently issued the "Special Action Plan for Patient Safety (2023-2025)" (hereinafter referred to as the "Plan"). The "Plan" requires medical institutions at or above the second level nationwide to further improve the patient safety management system, improve institutional construction, smooth work mechanisms, timely eliminate various risks in the medical process and hospital environment, minimize unnecessary harm to patients during the hospital period, and ensure patient safety over a period of three years. For three consecutive years, at least one round of hospital inspections and special training on patient safety has been completed annually. By the end of 2025, the level of patient safety management has been further improved, with more than 2.5 adverse events reported annually per 100 discharged patients. The mortality rate of low-risk inpatients has further decreased. In terms of safety management in inspection and testing, the "Plan" requires strict control of the indications and contraindications of various inspection and testing items, with a focus on high-risk, time-consuming or special requirements such as painless examination, endoscopic examination, invasive examination, exercise treadmill test, magnetic resonance imaging, enhanced imaging or imaging. Before conducting the examination, verify the patient's condition to ensure that they are prepared as required; During the examination process, closely observe the patient's condition, promptly detect and handle unexpected situations; After the examination is completed, guide or escort the patient back according to their actual situation. In the process of various diagnosis and treatment services, the "Plan" requires strict control of contraindications and indications, strict implementation of the check-up system, prevention of errors in the diagnosis and treatment objects, parts, and measures, and special attention to the situation of pregnant women, children, the elderly, patients with mental or consciousness disorders, and other special groups. Actively carry out full process management of medication, ensure the accuracy of the time, route, dosage, etc. of administration, prevent contraindications to drug use, compatibility, drug leakage, and promptly handle adverse reactions such as allergies, vomiting, and pain. In addition, the "Plan" also requires strict implementation of the core system of medical quality and safety, strengthening the evaluation and observation of hospitalized patients, paying attention to the mental and psychological status of patients, timely detecting and handling changes in patients' conditions, and preventing patients from falling off the bed, falling, getting lost, self harm, suicide, or harming other personnel. Strengthen the management of outpatient (emergency) patients and establish measures to identify high-risk patients and prioritize treatment. Strictly implement hospital infection management requirements, standardize the implementation of deep vein thrombosis prevention, reduce adverse events such as hospital infections and deep vein thrombosis, and achieve full coverage monitoring management for high-risk patients receiving surgical treatment, central venous catheterization, ventilator-assisted breathing, and long-term bed rest.
