Urgent Prevention and Treatment of "Young Early onset Coronary Heart Disease"

2023-04-24

Epidemiology has confirmed that coronary heart disease is common in the elderly, smoking males, and postmenopausal females. However, in recent years, clinical data shows that the incidence of coronary heart disease in China has shown a younger trend, with the incidence of acute coronary syndrome in the age group under 50 increasing by 30.3% in the past three years. The Research on Clinical Characteristics and Genetic Characteristics of Coronary Heart Disease in Young People under 45 in China (GRAND Study for short), presided over by Professor Ge Junbo, an academician of the CAS Member and the cardiologist of Zhongshan Hospital affiliated to Fudan University, found that the statistical data of young cases of coronary heart disease in the past six years showed that the incidence of acute coronary syndrome increased at a rate of more than 15% every year. The GRAND study has become the largest prospective, multicenter clinical research project in the world that focuses precisely on young coronary heart disease patients under the age of 45. It analyzes the relevant genes, susceptibility factors, clinical characteristics, and intervention methods of young Chinese patients with early-onset coronary heart disease from three dimensions: clinical, genomics, and metabolomics, and their impact on long-term prognosis. 38 domestic hospitals and research centers participated in the GRAND study, and more than 2000 young patients with coronary heart disease were enrolled with one arm. The research centers include Zhongshan Hospital affiliated to Fudan University, People's Hospital of Guangdong Province, Central Hospital of Karamay, Xinjiang, and the Second Hospital of Jilin University, basically covering the southeast, northwest, and central regions of China. In July last year, the authoritative comprehensive English academic journal "National Science Review" published the "Genomic Characteristics of Early onset Coronary Artery Disease in the Chinese Population Research Report", which proposed for the first time internationally that rare mutations in the LDLR gene and QTRT1 are pathogenic genes for early onset coronary heart disease in Chinese people. Metabolomics data is expected to be released within the year. Coronary atherosclerotic heart disease is a complex disease caused by environmental factors and genetic factors. Compared with elderly patients with coronary heart disease, young patients have distinct clinical characteristics and fewer traditional risk factors for coronary heart disease. However, the onset of the disease is sudden, and most of them manifest as acute coronary syndrome. The mortality rate outside the hospital is extremely high, and there are significant differences in the severity of vascular disease, intervention methods, and long-term prognosis compared to elderly patients. Therefore, the prevention and treatment of coronary heart disease, especially early onset coronary heart disease in young people, is urgent, and the role of prevention in health is far more important than treatment. Changing the bad diet, proper exercise and bad living habits are important drivers to reverse the high incidence rate of coronary heart disease. (New News Agency)
