The child has repeated high fever? Watch out for "heartbreak" of Kawasaki disease

2023-02-06

In winter and spring, the weather is sometimes cold and sometimes hot. Many children are prone to catch cold and fever. However, if the child's high fever persists, his body also has a rash, his eyes are also red, his lips and oral mucosa are congested and chapped, and his neck feels swollen lymph nodes, parents should be careful. The doctor reminded that this is probably not a common cold, but Kawasaki disease. If the treatment is not timely, the coronary artery may be damaged. Hu Jingjing, deputy chief physician of the Children's Rheumatology and Immunology Department of Guangdong Provincial Maternal and Child Health Hospital, said that Kawasaki disease is an acute febrile and eruptive childhood disease with systemic vasculitis as the main lesion, which mostly occurs in children under 5 years old. At the initial stage, it is very similar to upper respiratory tract infection, and can have fever, cough and other similar symptoms of upper respiratory tract infection, which is easily mistaken for cold and fever. Fever is the most common and earliest symptom of Kawasaki disease. It can cause high fever of 39 ℃ - 40 ℃. Without treatment, the fever can last for 1-2 weeks. During the fever period, parents can also find that the child has pleomorphic skin rash, red and swollen BCG vaccination site, conjunctival congestion, red lips, chaps, strawberry tongue, swollen neck lymph nodes, erythema on the palms and soles of feet, swelling at the ends of fingers (toes), peeling and other manifestations. Hu Jingjing reminded that there is another kind of incomplete Kawasaki disease, which is more hidden. The above typical symptoms appear or do not appear, which is often prone to misdiagnosis and missed diagnosis. The possibility of Kawasaki disease should also be considered when the child has fever for a long time and is accompanied by the following conditions: (1) the baby is irritable within 6 months; (2) Infants have aseptic meningitis of unknown cause; (3) Unexplained shock; (4) Antibiotic treatment of cervical lymphadenitis was ineffective; (5) Antibiotic treatment for pharyngolaryngeal wall and parapharyngeal cellulitis was ineffective. Although Kawasaki disease is self-limiting, it can also cause great harm if it is not treated in time. The most "dangerous" is coronary artery disease. It is reported that when Kawasaki disease in children is not treated in time, the incidence of coronary artery disease reaches 20% - 25%. If there is a coronary artery aneurysm, especially a huge coronary artery aneurysm, there is a potential risk of myocardial ischemia, myocardial infarction, and even sudden death. In addition, it can also have bronchitis, interstitial pneumonia, arthritis, arthralgia, nervous system involvement and other manifestations. If we can get timely diagnosis and treatment in the early stage of the disease, most of them can still recover and have a good prognosis. Hu Jingjing stressed that parents should keep in mind the symptoms and characteristics of Kawasaki disease. Children with fever should pay close attention to the changes of the disease. Once suspected symptoms of Kawasaki disease occur, they should be sent to the hospital in time, actively cooperate with treatment and do a good follow-up. (Outlook New Era)
