A systematic interpretation of the pathogenesis of sepsis immunosuppression

2022-10-13

The disease is dangerous and the case fatality rate is high. There are more than 18 million severe sepsis cases worldwide every year, but the underlying pathogenesis is still unclear. On the 12th, the reporter learned from the Army Characteristic Medical Center of the Army Military Medical University that the long review of Sepsis Immunosuppression: Pathogenesis and Diagnosis and Treatment Status, which was jointly studied by Academician Jiang Jianxin of the Army Military Medical University and several university teams, was recently published in the international military medical journal Military Medical Research. Sepsis is a systemic inflammatory response syndrome caused by bacteria and other pathogenic microorganisms invading the body. The incidence of sepsis is high, with more than 18 million severe sepsis cases worldwide every year. Sepsis is a dangerous disease with a high mortality rate. About 14000 people worldwide die of its complications every day. In recent years, despite considerable progress in anti infection treatment and organ function support technology, the mortality of sepsis is still as high as 30% - 70%. Sepsis treatment costs a lot and consumes a lot of medical resources, which seriously affects the quality of human life and has posed a huge threat to human health. At present, the clinical treatment for sepsis is mainly symptomatic treatment with broad-spectrum antibiotics combined with fluid resuscitation and organ function support. Although these treatment methods have greatly improved the hospital survival rate of patients, patients often return to hospital because of secondary infection after discharge, and the mortality rate of one year after discharge is as high as 44.3%. The reason is that the early inflammatory damage and secondary damage of sepsis lead to the patient's persistent immunosuppression, which makes the patient vulnerable to conditional pathogens, leading to secondary infection. This study systematically discussed the pathogenesis, diagnosis strategy and clinical treatment frontier progress of sepsis immunosuppression, which provided an important system basis for sepsis diagnosis and treatment in China, and was of great significance for improving the long-term prognosis of sepsis patients and improving their quality of life outside hospital.
