Make the family bed flat and stable

2023-02-16

Recently, Wuxi City, Jiangsu Province, issued the implementation opinions to speed up the construction of family hospital beds and provide home-based medical services for eligible patients. The expenses incurred will be reimbursed by medical insurance according to the hospitalization standard. As a local practice, Wuxi's exploration has enriched the national sample database of family bed reform. According to media reports, China's first smart home ward was launched in Guangzhou. The ward uses new technologies such as the Internet of Things and 5G to deploy smart wearable devices, smart home, and health monitoring equipment to the home, and is expected to realize the integration of smart home care, home care and home rehabilitation. Family ward is the extension of medical services such as diagnosis, treatment, nursing and rehabilitation of professional medical institutions. In this scenario, the family will become the main place for medical services, receiving diagnosis and guidance from professional institutions. This is undoubtedly a good news for those patients who have lost the ability of active behavior or are old and infirm. They can not only get treatment conveniently, but also stay in a familiar and warm family environment, instead of rushing on the road and being crowded in the hospital. Especially with the improvement of the aging degree of our society, more and more elderly people continue to have a growing demand for continuous medical treatment. For example, people with moderate and severe disability, patients with stroke and paralysis, patients with advanced malignant tumors, and patients with fracture traction who need to be treated in bed, all need to stay in bed for a long time. A good resting environment is particularly important for them. However, the current medical supply is still difficult to fully meet this demand, and the "difficulty in seeing a doctor" and "difficulty in hospitalization" are still prominent contradictions. Therefore, it is an important way to promote the construction of family sickbeds, so that more patients can receive professional diagnosis and treatment and care without leaving home, which can not only improve the well-being of patients and families, but also respond to the aging population, comprehensively strengthen elderly health services, and build a healthy China. However, from the perspective of local practice, there are still some factors restricting the development of family beds. At present, the overall supply of family beds is too small to meet the huge number of patients. On the one hand, low remuneration and high risk lead to low enthusiasm of hospitals; On the other hand, medical insurance reimbursement has become a big threshold in front of patients. Although a number of notices have been issued at the national level, requiring the construction of family beds to be vigorously promoted. For example, in December 2020, the National Health Commission and the National Administration of Traditional Chinese Medicine issued the Notice on Strengthening Home-based Medical Services for the Elderly; In December 2021, the National Health and Health Commission, the National Office for Aging and the National Administration of Traditional Chinese Medicine issued the Notice on Strengthening Health Services for the Elderly in an All-round Way. However, from universal advocacy to extensive implementation, more specific, more standardized and more thorough system design is needed. Medical service is a matter of conscience. Of course, inclusive services should be provided, but relevant departments should consider more about income, evaluation, promotion and other aspects to stimulate the enthusiasm of medical staff. In addition, the current medical insurance reimbursement policy based on medical treatment in hospitals also needs to be established, which requires specific regulations to be issued by all regions. This time, Wuxi City proposed that "the expenses incurred will be reimbursed by medical insurance according to the hospitalization standard", opening up a new way for reference. Furthermore, the design of a family hospital bed should also be consistent with the current hierarchical diagnosis and treatment
