Shanghai hospital explores innovative measures to improve the diagnosis and treatment of lymphedema

With the increasing incidence rate of breast cancer, the number of patients receiving surgery for breast cancer is also increasing. After surgery, a considerable number of patients may suffer from lymphedema, with symptoms such as chronic edema, skin tightening and fibrosis, heavy limbs, pain, and chronic inflammation. Due to technological limitations, the treatment of lymphedema mainly relies on conservative treatments such as medication and physical therapy, but the results are not ideal. In recent years, with the improvement of super microsurgery technology and fluorescence lymphatic imaging technology, the application of super microscopy technology for lymphatic vein anastomosis has shown good clinical results. Over the years, Yang Huafeng, the chief physician of the nail and breast surgery department of Punan Hospital, led the team to explore and research the surgical treatment of lymphedema, and took the lead in carrying out the ultramicroscopic lymphatic vein anastomosis operation in the city to treat upper limb lymphedema after breast cancer surgery. The team used a high-powered surgical microscope to magnify the surgical field by more than 35 times, carefully searching for functional lymphatic vessels and tiny superficial veins that are thinner than a hair. Through super microsurgical techniques, the two were anastomosed to re-establish the lymphatic return pathway, fundamentally solving the cause of lymphedema for patients. At present, lymphatic vein anastomosis has become the standard surgical treatment for lymphedema. Aunt Chen from Xiamen underwent modified radical surgery for left breast cancer 2 years ago, and the subsequent lymphatic reflux disorder in her left upper limb seriously affected her life, causing her unbearable pain. In August of this year, Aunt Chen learned that Professor Yang Huafeng's team had a mature treatment model for "lymphedema" and went to Punan Hospital for treatment. Yang Huafeng immediately arranged for Aunt Chen to be hospitalized, and after completing the examination, performed lymphatic vein anastomosis under a super microscope. The surgery took 4 hours, during which a total of 18 lymphatic vessels were anastomosed. Three days after the surgery, Aunt Chen felt a significant reduction in swelling in her left upper limb, and the long lost feeling of relaxation in her left upper limb returned. Yang Huafeng stated that the lymphatic system is an important component of the human immune system. It acts as a "drainage pipe" throughout the body, responsible for collecting tissue fluid and returning it to the bloodstream. Lymph contains immune cells that can resist bacterial infections and also clear waste and toxins. Upper limb lymphedema is caused by the inability of lymphatic fluid to flow back normally, leading to the accumulation of upper limb tissue fluid and causing upper limb swelling. In the early stage, the area of lymphedema is relatively soft and concave, which gradually deteriorates, leading to numbness and pain in the affected limb. In severe cases, upper limb dysfunction, sensory abnormalities, and even limb deformities may occur. In addition to upper limb lymphedema caused by breast cancer, lower limb lymphedema often occurs after gynecological tumors such as cervical cancer, endometrial cancer and urinary system tumors, which is also suitable for ultramicrosurgical treatment. Since the completion of the first ultra microscopic lymphatic vein anastomosis surgery by Yang Huafeng's team in September last year, more than 60 cases of limb lymphedema have been treated with good surgical results so far. In order to promote awareness of lymphedema among all sectors of society and improve the treatment level of lymphedema in the medical profession, the "First lymphedema Diagnosis and Treatment Expo Forum" hosted by Punan Hospital was successfully held in September this year. Professor Jaume Masia, the President of European lymphedema, delivered a video speech at the conference. The conference invited more than 20 domestic experts in fields such as surgery, nursing, physical rehabilitation, and lymphedema treatment to engage in academic exchanges on cutting-edge developments, precise diagnosis, precision surgery, full process management, prevention and rehabilitation of lymphedema.
