Assisted reproduction included in medical insurance to accelerate and expand coverage

The inclusion of assisted reproduction in medical insurance is an important measure of medical insurance policies to promote the construction of a fertility friendly society. As of the end of 2023, four provinces including Beijing, Guangxi, Gansu, and Inner Mongolia will include assisted reproductive technology in medical insurance reimbursement. In March of this year, Zhang Ke, Director of the National Medical Insurance Administration, conducted a survey and stated that he will continue to guide areas with conditions to further improve the establishment of assisted reproductive medical services and medical insurance payment management, and strengthen departmental coordination to promote the connection between basic medical insurance, maternity insurance, and related economic and social policies, and jointly promote long-term balanced population development. Subsequently, medical insurance departments in multiple provinces actively followed up. As of June 1, 2024, six provinces including Shanghai, Zhejiang, Jiangxi, Shandong, Qinghai, Xinjiang Uygur Autonomous Region, and Xinjiang Production and Construction Corps have also included assisted reproduction in their medical insurance reimbursement scope. The inclusion of assisted reproduction in medical insurance is accelerating its expansion. In order to standardize the treatment service projects and prices in the industry, promote the standardized and efficient development of related medical service projects, in June 2023, the National Medical Insurance Administration issued the "Guidelines for the Approval of Auxiliary Reproductive Medical Service Price Projects (Trial)", which classified and integrated the originally diverse auxiliary reproductive projects in various regions into 12 items, such as egg retrieval, embryo culture, embryo transfer, artificial insemination, etc. As of now, 24 provinces have been guided to integrate assisted reproductive projects in accordance with the project approval guidelines, and 23 provinces have changed their pricing methods from market adjusted prices to government guided prices. After implementing government guided pricing, costs have been reduced to varying degrees. The average price reduction in Inner Mongolia after adjustment reached 20%. Jiangxi has scientifically formulated project prices through various methods such as inter project comparison, inter disciplinary comparison, and inter regional comparison. Taking the price of provincial hospitals in Nanchang as an example, "egg retrieval surgery" has been reduced from 1500 yuan to 1000 yuan, and "embryo transfer" has been reduced from 1600 yuan to 1000 yuan. In addition, different regions have different regulations on guidance prices based on actual situations, for example, Guangxi, Qinghai, Shandong, Xinjiang, and the Xinjiang Production and Construction Corps implement fixed guidance prices; Shanghai adopts a maximum fee limit model to encourage medical institutions to strengthen management and control costs. Assisted reproductive technology has fulfilled the fertility dreams of many families with family planning. But there are also many families who are hesitant due to their high prices. Yang Yihua, Deputy Director of Guangxi Reproductive Medicine Research Center at the First Affiliated Hospital of Guangxi Medical University, introduced that the cost of assisted reproductive transplantation cycle treatment is 30000 to 40000 yuan, and more than half of women need to receive assisted reproductive treatment again or even multiple times. After being included in medical insurance reimbursement, the burden will be greatly reduced. Starting from November 1, 2023, Guangxi will include 9 auxiliary reproductive medical service projects in the payment scope of basic medical insurance and work-related injury insurance funds, without setting a minimum payment standard. The reimbursement ratios for employee basic medical insurance and urban and rural resident basic medical insurance are 70% and 50%, respectively, and will be included in the maximum annual fund payment limit for insured persons. According to data from the Guangxi Human Assisted Reproductive Technology Management Center, since the inclusion of assisted reproductive technology in the medical insurance reimbursement policy for more than half a year, the local assisted reproductive treatment volume has significantly increased, with 993700 outpatient visits in assisted reproductive institutions in the entire autonomous region, a year-on-year increase of 14.74%. Starting from June this year, Jiangxi will include 9 projects such as "egg retrieval surgery" and "artificial insemination" in the scope of medical insurance payment. insured personnel will be reimbursed according to the Class B reimbursement ratio according to hospitalization regulations. According to the number of infertile women of childbearing age in the local area and their willingness to implement, it is expected that within one cycle of assisted reproductive transplantation, the burden of medical expenses on insured individuals will be reduced by 290 million yuan, effectively promoting long-term balanced population development.
