Beware of sporadic loss of medical insurance funds in retail pharmacies
2024-06-05
Recently, the Fund Supervision Department of the National Medical Insurance Administration held an interview with the relevant person in charge of Yixintang Pharmaceutical Group Co., Ltd. (referred to as "Yixintang"). During the interview, it was pointed out that the medical insurance department found in the fund supervision work that some designated chain stores under Yixintang have problems such as colluding with others to exchange drugs, prescribing excessive amounts of drugs, designated retail stores that temporarily suspend medical insurance settlement, mismatched drug purchase, sales, and inventory records, and non-standard prescription drug sales, resulting in losses to the medical insurance fund. The issue pointed out in this interview is a common problem in some retail pharmacies. "This medicine cannot be used to swipe your medical insurance card. I will replace it with another one." "This medicine requires only two bottles to be opened at a time. If you need 10 bottles, I will label it as another medicine on the list." "It doesn't matter if you don't have a prescription, I will find a way to make it up for you." "Although our medical insurance settlement qualification has been suspended here, we can go to other pharmacies to swipe your card for you." I believe many people have heard this kind of statement from pharmacy staff when buying medicine at pharmacies. ". This operation provides customers with benefits and convenience, but it secretly leads to the loss of medical insurance funds. The key is that compared to medical institutions, there are significant difficulties in regulating the medical insurance fund for pharmacies. Medical institutions have medical records and prescriptions, have rigorous diagnostic and treatment strategies, and are prone to leaving marks when dealing with medical insurance funds. However, the entry and exit of drugs in retail pharmacies may be a confusing account, with prescriptions being prescribed as they please. Medication does not have the constraints of diagnosis and treatment ideas and medical records. In addition, the concealed and fast drug sales process, as well as the large number and wide distribution of retail pharmacies, make it easy for medical insurance funds to experience sporadic loss, and it is even more difficult to discover clues and obtain evidence. Pharmacy is one of the "dual channel" guarantee channels. With the continuous deepening of medical reform, more medical insurance negotiated drugs will be sold through pharmacies; Patients who purchase medication at designated pharmacies under medical insurance can enjoy the same reimbursement treatment as hospitals; With the acceleration of medical informatization construction, it will become more common to prescribe prescriptions in hospitals and take medication from pharmacies... These objectively may increase the difficulty of regulating pharmacies. The medical reform has expanded the channels for pharmacies to sell medical insurance drugs. In some places, there is also a trend of frequent drug exchange and drug reselling. Compared with individual pharmacies, chain pharmacies have more stores, complete drugs, and are more convenient for inter store exchange and settlement. Therefore, chain pharmacies should become the top priority for the next step of supervision. It should be noted that drug buyers are also prone to becoming accomplices to pharmacy fraud and insurance fraud. The prescription of drugs that cannot be prescribed and the reimbursement of drugs that cannot be reimbursed have benefited individuals, and the pharmacy has also made money. The two have shared interests, which can easily form a tacit understanding of keeping secrets, making it more difficult for outsiders to notice. But once such "cooperation" among drug buyers is discovered, they may face punishment from medical insurance, and in severe cases, they may face legal punishment. Drug buyers should have a clear understanding of this. A chain pharmacy being interviewed may seem like a small matter, but it has obvious signs and trends. The National Medical Insurance Administration has specially announced this matter, which has obvious warning significance. The medical insurance department, retail pharmacies, and buyers should all recognize that, given the widening channels for pharmacies to illegally use medical insurance funds, the threat to medical insurance funds is continuously increasing; Only by including pharmacies in the key regulatory list, correcting their development concepts and strengthening internal management, and ensuring that drug buyers do not participate in insurance fraud and are good at monitoring and reporting, can pharmacies, as a channel for the loss of medical insurance funds, be blocked and further protect the "life-saving money" of the people.
