How does the "sixth insurance" of social security reduce the burden on Chinese families?

"Promoting the establishment of a long-term care insurance system" has been included in this year's Chinese government work report. Entering an aging society, how can this social security "sixth insurance" that focuses on caring for disabled elderly people reduce the burden on Chinese families? Long term care insurance refers to insurance that pays fees and provides services to disabled individuals who have lost their ability to take care of themselves due to illness, aging, or disability and have to stay at home or medical institutions for a long time. It is known as the "sixth insurance" in addition to the five social insurances of elderly care, medical care, work-related injury, unemployment, and childbirth. According to data cited by Qin Haitao, a member of the National Committee of the Chinese People's Political Consultative Conference and Vice Chairman of the Jilin Provincial Committee of the Chinese People's Political Consultative Conference, as of the end of 2023, the number of elderly people aged 60 and above in China has reached 297 million, accounting for 21.1%. According to a survey, there will be approximately 44 million disabled and semi disabled elderly people in China in 2022, and this number will rise to 72.79 million by 2025. The proportion of the elderly population is expanding, and how to avoid "one person being disabled and the whole family being imbalanced" has become a topic of concern for many families. Yu Xubo, a National People's Congress representative and Chairman of General Technology Group, believes that comprehensively promoting the implementation of the long-term care insurance system can not only reduce the burden on families, but also to some extent avoid "poverty caused by care" and reduce the long-term dependence of chronic diseases on hospitals. In 2016, 15 cities in China launched pilot programs for long-term care insurance, which have now expanded to 49 cities. As of the end of 2022, the number of long-term care insurance participants in China reached 169 million. "Compared to the size of the disabled population, the overall number of long-term care insurance participants is still relatively small and urgently needs to be expanded," said Huang Hongxia, a member of the National Committee of the Chinese People's Political Consultative Conference and director of the Henan Provincial Health Commission. But where the funds for long-term care insurance come from is still a problem that needs to be solved. Liu Juncai, member of the National Committee of the Chinese People's Political Consultative Conference and director of the Beijing Municipal Health Commission, said that currently, the funding for long-term care insurance in pilot cities relies heavily on medical insurance funds and has not formed an independent and stable source of funding. In response to the current situation of narrow financing channels, Qin Haitao suggests that residents participate in insurance through a combination of personal contributions and financial subsidies to raise funds. Explore the classification of funding standards based on different conditions such as occupation and age, and establish a dynamic adjustment mechanism that is suitable for the disposable income and social security level of urban and rural residents. Yu Xubo suggested that long-term care insurance could be included as an independent insurance type in the Social Insurance Law, promoting the standardization, legalization, and regularization of long-term care insurance. At the same time, establish a unified and standardized financing mechanism, optimize the use of personal account funds for medical insurance, coordinate management, and intensively use various national funds related to the elderly, etc., to reserve sufficient funding sources for long-term care insurance. Reducing the burden of family care is the core goal of long-term care insurance. Based on an estimated 72.79 million disabled or semi disabled individuals by 2025, China will need tens of millions of elderly care workers in the future, with a significant gap. Liu Juncai believes that it is necessary to strengthen the construction of the elderly care service industry, attach importance to the integration of medical and elderly care, promote the transformation of some primary and secondary hospitals in areas with abundant medical resources into nursing and rehabilitation hospitals, and use market-oriented mechanisms to drive social capital investment; And explore the establishment of a system based on social long-term care insurance and supplemented by commercial long-term care insurance, so that commercial long-term care insurance can meet the multi-level nursing needs of the public.
