Expert: Childhood depression ≠ Adult depression or induced drowsiness symptoms

2023-10-13

October 10th is World Mental Health Day. Sleep disorders are often common comorbidities of mental disorders. Pan Jiyang, a professor at the Department of Psychiatry and Sleep Medicine Center of the First Affiliated Hospital of Jinan University, stated that, unlike adult depression, childhood depression exhibits unique symptoms that require the attention and early identification and intervention of parents, educators, and medical experts. Parents and teachers should pay attention to children's unfounded tantrums, irritability, and inexplicable physical discomfort. Tong Tong (pseudonym), who is 12 years old this year, has been an excellent student since childhood, but Tong Tong, who is in the sixth grade of elementary school, has recently developed a strange symptom - drowsiness, feeling like he can't wake up. At first, the teacher felt that Tong Tong's schedule was irregular and he played with his phone at night, so he asked his parents to discipline him well. However, Tong Tong said that he did not intentionally sleep in class, but could not control his drowsiness. After evaluation by the Sleep Clinic of the Department of Psychiatry at the First Affiliated Hospital of Jinan University, doctors found that Tong Tong had depression. Under the doctor's inquiry, Tong Tong said that he often hides and secretly cries, believing that his parents have not loved him since they had a younger brother. He has a lot of academic pressure and feels inferior to others. He claims that his life is meaningless, and he has repeatedly used his hands to pinch his neck and bump his head to hurt himself. To further clarify Tong Tong's sleep problems, doctors suggest that Tong Tong undergo nocturnal polysomnography (PSG) and multiple sleep latency tests (MSLT) at the Sleep Medical Center of the First Affiliated Hospital of Jinan University, in order to further clarify the diagnosis. Pan Jiyang considered Tongtong's presence based on his clinical manifestations and relevant auxiliary examination results: 1. Severe depressive disorder; 2. Suspected narcolepsy. As for the current situation, it is recommended to maintain a normal daily routine and undergo medication treatment, mainly to treat the underlying emotional problems, and sleep problems will also improve accordingly. Childhood depression is a common but often overlooked mental health problem, "said Pan Jiyang. The precursors of childhood depression may not necessarily be low mood or sadness, but rather anxiety, irritability, or non organic physical symptoms. In addition, in most people's cognition, insomnia is closely related to depression, but excessive sleep is also associated with depression. Excessive daytime sleepiness is often considered a result of sleep disorders or insufficient sleep. This manifestation is related to various sleep disorders, such as insomnia, sleep snoring, circadian rhythm sleep disorder, and restless leg syndrome. However, in fact, it is most commonly associated with psychological disorders, especially depression disorder. Pan Jiyang said that excessive sleep is an atypical manifestation of depression, which is more common in young people (40% of depression patients under the age of 30, 10% of depression patients under the age of 50) and women. The main manifestations include daytime drowsiness, Feeling unable to regain energy or alertness after daytime rest, but excluding nighttime sleep. This phenomenon is related to long-term, severe, and refractory depression, and may also indicate potential bipolar disorder. Therefore, early identification and treatment are important guarantees for improving patients' sleep problems and prognosis. Author: Cai Minjie, Zhang Cancheng (from Xinshe)
