In June 2013, Sanming took the lead in establishing the medical security fund management center of "three guarantees in one". From medical insurance to medical security, the medical insurance department has changed from handling medical insurance business in the past to the key to the reform of "three medical linkage" of medicine, medical insurance and medical treatment. In July 2016, Sanming took the lead in comprehensively integrating the responsibilities of human resources, social security, health care, civil affairs, price and so on, and established the first medical security bureau in China. I was honored to be the first director of medical insurance in China. After the establishment of the National Medical Insurance Bureau in 2018, we focused on the key point of centralized procurement of drugs and consumables, and carried out medical insurance access negotiations and procurement of drugs and consumables in a regular manner. The prices of a large number of high-quality drugs and high-value consumables decreased significantly. Many expensive anti-cancer drugs were also included in the medical insurance reimbursement, making "life-saving drugs" affordable. Sanming City immediately followed up and implemented the results of national negotiations and national and provincial centralized procurement of pharmaceutical consumables, so that the insured people would feel full of sense of gain and happiness. For example, before the medical reform, the commonly used drug for diabetes patients, the imported acarbose tablets produced by Bayer, Germany, was about 120 yuan/box, which was reduced to 91 yuan/box after Sanming implemented the joint limited price purchase of drugs. After the centralized purchase by the National Medical Insurance Bureau, the price of this drug has dropped to 9.21 yuan/box. Since 2015, Sanming has adhered to the concept of health as the center, providing 39 basic drugs free of charge for patients with six chronic diseases, such as diabetes and hypertension. At present, it has included imported acarbose tablets into the scope of free drug use, that is, there is no starting and paying line, and the medical insurance will reimburse 100%. Since the implementation of the free drug policy, the city has provided 2.6672 million person times and 38.8693 million yuan for free. In the past ten years of Sanming medical reform, the proportion of inpatient reimbursement of urban and rural residents in public hospitals above county level in the overall planning area has increased from 46.25% in 2011 to 67.57% in 2021. Sanming, however, is still exploring the way to deepen medical reform, adhering to the concept of "people first, health first", building a new era health security system, and making efforts to make people feel more secure and happy.
