This is my first impression of seeing the snow covered plateau when I first visited Ganzi Tibetan Autonomous Prefecture of Sichuan Province with the hospital in 2006. It's so pure and beautiful. However, under the snow capped mountains, many Tibetan people are deeply troubled by echinococcosis. At that time, the economy of areas with high incidence of hydatid disease was relatively backward, the income of farmers and herdsmen was generally low, and the awareness of health and disease prevention was weak. Many farmers and herdsmen suffering from echinococcosis are in advanced stage when they seek medical treatment, and their livers are seriously eroded. Local people turn pale when talking about the disease and think it is an "incurable disease". There are often cases of illness in the whole family, and many farmers and herdsmen give up treatment after learning that they are ill. At that time, I secretly decided that as a liver surgeon, I must do something for these kind and honest Tibetans and find ways to help them free from the torture of liver hydatidosis. Sink advanced medical technology into epidemic prone areas Most of the local patients are alveolar echinococcosis, and surgery is the main treatment. Especially in the late stage, they can only be saved by liver transplantation, while the cost of traditional liver transplantation is at least 500000 yuan, and they also need to take expensive anti rejection drugs for life after operation, which is an unrealistic treatment plan for these already poor families. We found in clinical practice that many patients with echinococcosis in the late stage, although most of the liver has been seriously eroded, there is still a small part of the liver with normal structure can be used. We have broken through many technical bottlenecks and explored an effective treatment, that is, in vitro hepatectomy combined with autologous liver transplantation. In 2014, we successfully completed the first autologous liver transplantation for advanced alveolar echinococcosis in Sichuan Province in West China Hospital of Sichuan University. At that time, it took me and more than a dozen doctors in five departments 18 hours to successfully complete the operation. After the successful operation, I repeatedly thought about how to sink the advanced medical technology into the epidemic high-risk area and effectively relieve the disease problems for the Tibetan people. At that time, the hospital in Ganzi could only do simple cystic echinococcosis surgery, and the slightly more complex alveolar echinococcosis could not be treated at all. "It is better to teach people to fish than to teach people to fish". I am determined to go deep into Tibetan areas to carry out medical assistance. I went to Ganzi Tibetan Autonomous Prefecture People's Hospital for ward rounds, lectures, clinical norms, on-site teaching surgery, and hand-in-hand training of professionals for local hospitals. We also check the treatment of every patient through the remote consultation platform and 5g ward round equipment. Relieve Zeren's pain in the operating room at an altitude of 2800 meters In 2016, I consulted a Tibetan female patient named Zeren on the remote network consultation platform of West China Hospital of Sichuan University and the people's Hospital of Ganzi Tibetan Autonomous Prefecture. Three of the five people in my family have suffered from liver hydatidosis, and my father and sister both died of liver hydatidosis. At that time, she was in the late stage of the disease and had no economic conditions to see a doctor in Chengdu. After many remote consultations and plan drills, we decided to go to Ganzi Tibetan Autonomous Prefecture People's hospital to carry out in vitro hepatectomy combined with autologous liver transplantation for this patient. We set out from Chengdu at 6 o'clock in the morning and rushed to the hospital at 1 o'clock at noon. After 15 hours of hard work, the operation ended in the early morning of the next day. The whole operation process is full of challenges, and the biggest difficulty is the physical maladjustment. The operating room at 2800 meters above sea level is stuffy and narrow. After wearing the mask, I feel more chest tightness and dizziness. I can only wear a long oxygen tube like the patient and finish the operation while inhaling oxygen. A month later, when Ze Ren stood in front of me with her 2-year-old daughter and bowed to us to thank hada, my heart as a doctor was full of a sense of achievement. This also made the majority of medical staff in the state feel extremely excited, and they rushed to tell each other, because this was an operation that they couldn't even think of in the past. At the beginning, I chose to arrange such a complex operation to the people's Hospital of Ganzi Tibetan Autonomous Prefecture with relatively simple equipment and conditions. In addition to considering the economic difficulties of patients, another important reason is that I want local doctors to participate in and witness such a complex operation, so as to enhance their confidence in learning complex hepatobiliary surgery technology. Countless times I have walked through national highway 318 and stepped on the natural danger under my feet For the solution of echinococcosis, surgical treatment is only the symptom, and prevention is the root cause. We and local medical teams go to many counties and townships in Ganzi Tibetan Autonomous Prefecture every year, go to schools and people's homes, and publicize disease prevention knowledge in the form of short videos, Tibetan songs, etc. Once, I and the medical team of the state hospital carried out hydatidosis publicity and follow-up in sexu Town, Shiqu County. Nearly a thousand farmers and herdsmen sat on the ground against the hot sun on the plateau. I gave them hydatidosis prevention and control knowledge publicity and hand hygiene knowledge demonstration. Just at the end of the propaganda, there was the roar of motorcycles outside the courtyard. It turned out that Ze Ren and her wife learned that the West China medical team had arrived in sexu town and rushed over from more than 100 kilometers away in the early morning, just to offer hada to us. Several herdsmen also came out of the crowd and joined the xianhada team. They were all patients who had undergone surgery in the people's Hospital of Ganzi Tibetan Autonomous Prefecture. Some of these simple farmers and herdsmen arrived at the meeting on horseback for dozens of kilometers, and some arrived in the town by car yesterday, waiting for us here. Looking at the enthusiastic and honest farmers and herdsmen, I can't understand their words, but I can feel their affirmation of our work, and I'm very proud of it. On the way back to Chengdu, this scene always came to my mind, which strengthened my confidence and determination to relieve the pain of farmers and herdsmen. In the past, when I went to Ganzi Tibetan Autonomous Prefecture, I passed national highway 318 countless times and stepped on the natural danger Erlang Mountain countless times. The six and a half hour journey was rough and difficult. After the operation, I often have to rush back to Chengdu overnight in order to catch up with the outpatient service and operation the next day. Now, thanks to the construction of remote areas by the state, Chengdu is connected to Ganzi by expressway, which can be reached in 3 hours. The drive is shorter, and the "heart distance" between me and the local farmers and herdsmen is closer. (outlook new era)
