The city will provide no less than 1800 beds for hospice care services, and community health service institutions can generally provide community and home hospice care services, The demand for hospice care services for the elderly will be basically met. According to the implementation plan, the city will accelerate the establishment of a comprehensive, continuous and connected hospice care service system based on community and home, supplemented by institutions. We will develop and build a number of hospice care institutions through transformation, new construction, reconstruction and expansion. By 2025, at least one hospice center will be established in each district to support community and home hospice services. The rehabilitation institutions, nursing institutions, community health service institutions and integrated medical care institutions providing services will be included in the construction of the medical consortium, so as to promote the sinking of high-quality service resources at the grass-roots level. Hospice centers mainly provide institutional services for patients who need hospitalization. Explore the establishment of multi-disciplinary teams such as doctors, nurses, rehabilitators, medical social workers, psychological counselors and nutritionists to jointly carry out the service mode, so as to provide comprehensive and whole process integrated services for patients. Tertiary hospitals mainly provide hospice care services for patients with sudden acute lesions or severe physical and psychological symptoms who need hospitalization, and undertake tasks such as technical support and professional training. Encourage tertiary hospitals to establish counterpart support and referral cooperation mechanisms with hospice centers, community health service centers and other medical institutions providing hospice services, so as to give full play to the assistance and driving role of high-quality medical resources. Community health service institutions, nursing institutions and elderly care institutions mainly provide institutions and home hospice care services for patients with clear diagnosis, mild and stable symptoms. If possible, services suitable for home can be carried out according to relevant requirements, including symptom control, comfortable care, psychological support and humanistic care, through the establishment of family sickbeds, patrol and other ways. Exploring Internet plus hospice care New service formats The implementation plan also proposes that the city will select a number of qualified medical institutions (including traditional Chinese medicine medical institutions) and community health service centers as Beijing hospice guidance center, hospice demonstration base and community hospice service demonstration center to undertake service demonstration guidance, quality control, publicity and education, scientific research innovation, talent training Discipline construction and other tasks. In the future, the city will also explore the establishment of a service model centered on dying patients and their families and multi-disciplinary team cooperation, provide pain and other symptom control, comfort care, psychological comfort and other services for patients with end-stage diseases, provide social support, psychological support, death education and humanistic care for patients and their families, and formulate service norms and standards. Actively explore the new format of "Internet plus hospice care", and improve the convenience of services through online booking, online follow-up, health consultation and smart medical equipment, and adapt to the needs of the elderly personalized services. Gradually, a variety of hospice care models such as medical institutions, communities and homes, medical care, Internet plus hospice care are gradually formed. In terms of improving the price policy, according to the implementation plan, for-profit medical institutions can determine the content and charging standard of hospice care services by themselves. Hospice care services provided by non-profit medical institutions, which belong to medical services such as treatment, nursing, inspection and inspection, shall be charged according to the existing items; Non medical services such as care and consolation and life care shall not be managed as medical service price items, and the charging standard shall be independently determined by the medical institution. In addition, actively explore diversified payment methods such as payment per bed day, and gradually implement personalized performance evaluation for hospice care institutions and departments to improve the enthusiasm of medical staff.
