Promote mutual recognition of inspection results, with policies and implementation

On November 27th, the National Health Commission and seven other departments issued the "Guiding Opinions", proposing specific measures to further promote mutual recognition of inspection and testing results among medical institutions from the aspects of strengthening institutional design, enhancing technical support, increasing policy support, and deepening work implementation. The Guiding Opinions clarify the task objectives for 2025, 2027, and 2030 respectively, and refine the task requirements for close medical consortia at the municipal, provincial, regional, and national levels such as Beijing Tianjin Hebei, Yangtze River Delta, and Chengdu Chongqing. As early as 2006, the original Ministry of Health issued a document proposing for the first time the mutual recognition of examination data and test results among medical institutions, which has been continuously mentioned in subsequent documents. This work was significantly accelerated in July 2021 when the National Health Commission issued the "Notice on Accelerating the Mutual Recognition of Inspection and Testing Results", followed by the joint issuance of the "Management Measures" by multiple ministries. Currently, many cities have carried out mutual recognition of inspection results. One of the highlights of the "Guiding Opinions" is the clear timeline and target tasks for achieving mutual recognition of inspection results. By the end of 2025, all projects within close medical consortia will be mutually recognized, with over 200 projects recognized within each city. By the end of 2027, there will be over 300 mutual recognition projects within each province; There are over 200 mutual recognition projects in the Beijing Tianjin Hebei region and other areas. By 2030, common examination and testing results will be shared and recognized across regions and medical institutions. According to this deployment, "mutual recognition and benefit for the people" is expected to become a reality as soon as possible. At the same time, the "Guiding Opinions" clarify the "roadmap", including the organization and release of mutual recognition project lists and medical institution lists within the jurisdiction of local health administrative departments; Strengthen the construction of regional information platforms and achieve cross institutional sharing and retrieval within the region; Strengthen the infrastructure construction of the provincial coordinated regional national health information platform, medical insurance information platform, provincial "image cloud", and network storage, and do a good job in the daily maintenance and data exchange of relevant information platforms. Of course, we also need to objectively consider the difficulty of mutual recognition of certain test results. For example, sometimes due to changes in the patient's condition, the existing test results are difficult to reflect the actual condition of the patient, and the receiving hospital has to rearrange the examination; Some patients or their families request further follow-up examinations, and the hospital cannot ignore the patient's request. Only a follow-up examination can give the patient a "reassurance pill". Due to objective reasons, these test results cannot be mutually recognized and need to be viewed rationally by the public. Creating favorable conditions for mutual recognition of test results is to avoid patients undergoing repeated examinations, which can also reduce the burden of medical treatment, save medical time, and improve medical efficiency. At the same time, this is also beneficial for saving money for the medical insurance fund and reducing service pressure for hospitals. So, from relevant departments in various regions to medical institutions, they should actively implement the specific deployment of the "Guiding Opinions". Not only should the number of mutual recognition projects be implemented on schedule, but also the quality of mutual recognition should be improved, striving to achieve a win-win situation for all parties. It should be pointed out that promoting mutual recognition of test results also requires preventing the practice of "using tests to support doctors". Due to high operational pressure, some medical institutions increase their revenue through repeated inspections and excessive laboratory tests, resulting in negative phenomena such as "too many inspections" and "expensive inspections". Therefore, in order to support mutual recognition of test results, it is necessary to effectively crack the chaos of "relying on testing to support medicine" and prevent individual medical institutions from "hollowing out" the policy of mutual recognition of test results through repeated or excessive testing. So, in addition to list support and technical support, it is also necessary to improve the assessment of medical institutions and force them to move away from "relying on medical examination and maintenance". This requires not only improving the subsidy mechanism for public hospitals, but also establishing a sound reward and punishment mechanism, closely monitoring the changes and obstacles in the burden of patient examination and testing after the implementation of mutual recognition of test results through big data and other means.
