By 2025, the health management rate of traditional Chinese medicine for the elderly aged 65 and above will reach 75%; Carry out the prevention and treatment of chronic diseases with traditional Chinese medicine, build the "hospital community individual" chronic disease management mode, and carry out the whole-cycle health management of chronic disease patients with traditional Chinese medicine. The Plan also proposes the promotion activities of the intervention plan for the prevention of diseases by traditional Chinese medicine, the activities of "Chinese medicine into the family", the intervention activities of traditional Chinese medicine for myopia, obesity and scoliosis in teenagers, the activities of medical and physical integration and strengthening, and the cultural dissemination activities of traditional Chinese medicine.
