In the past 10 years, I have witnessed the rapid development of orthopedic technology, and I have also seen that the reform of the medical system has greatly alleviated the problem of "difficult and expensive to see a doctor", and I have also seen Chinese orthopedic doctors shine brightly in the international orthopedic community. In 2011, the spine team in my ward first proposed the VCD osteotomy orthopedic technique for the treatment of severe spinal deformities in the world, which brought good news to patients, and the difficulty of the operation and the treatment effect also made orthopaedics departments in the world applaud it. Every year during the cold and summer, we receive a large number of small patients with scoliosis from all over the country. Most of them are scoliosis, lordosis and kyphosis, which are extremely difficult to treat. Some children not only have the whole scoliosis, but also rotate at the same time. It is conceivable that the operation is difficult, and the biggest risk is paraplegia caused by irreversible nerve injury. After the treatment of new technology, the children one by one stood up and walked down from the hospital bed, returned to society, and began a new life. It is not only the responsibility of our orthopedic medical workers, but also our honor to make them stand up again. It is this bit by bit moving that has accompanied my work in the past ten years; It is this precision that shows the high level of orthopedic medical treatment in China. In addition to the flowers of our motherland, we have also treated many elderly patients. Since 2002, we have been treating elderly patients with hip fractures, many of whom are over the age of 80 or 90 years old. It is not uncommon to treat long-lived patients over 100 years old with joint replacement surgery. On the first day after surgery, we helped the patients get out of bed and walk to recover their pre injury function, changing the traditional conservative treatment of lying in bed and self-healing, which is a major breakthrough in the treatment of elderly fracture patients. I remember an old man who felt that he would be a drag on his family after his fracture. He didn't want to open up for a moment and crawled to the bedside to take sleeping pills. After hospital rescue, they were admitted to our department for surgical treatment. With our help, the old man was able to get out of bed and walk on the first day after the operation. Her confidence and smile returned to her face, and she gave us a thumbs up. My department began to introduce Mako robot assisted total hip arthroplasty in 2018, and then carried out total knee arthroplasty. Robot assisted joint replacement is a cutting-edge technology in the academic field of joint replacement, which can greatly improve the accuracy of prosthesis implantation position. At the same time, due to the implementation of the national centralized procurement system of drugs and medical devices, the average price of materials has been reduced by about 90%, so that the majority of patients with arthropathy can no longer endure the pain and receive treatment in a timely manner. In the past 10 years, I have also seen the rapid progress of minimally invasive surgery technology. Lumbar intervertebral disc herniation, a common and frequently occurring disease, has been well known by patients as an effective minimally invasive treatment. In the original lumbar surgery, the knife edge was at least 10 cm, and the patient could not get out of bed 4 days after the operation. Nowadays, the surgical incision of lumbar intervertebral foramen mirror is only 6mm, and patients can walk under the operating table with the help of doctors after operation. Without hospitalization or infusion and injection, you can go home on the day of operation, and the medical cost is reduced by 2/3, which greatly saves medical resources for the country and greatly reduces the treatment burden for patients. 36 years ago, I have been engaged in orthopedic clinical work since I graduated from medical school. My department has developed from the initial two wards to the maximum of nine wards, and from the comprehensive orthopedic ward to five sub specialties (trauma, spine, joints, sports medicine, bone tumors). In this process, our medical workers have never stopped pursuing the progress of technology and services. For example, ultrasound technology can display soft tissue structures such as muscles, tendons, ligaments, nerves and blood vessels. When applied to the puncture process of lumbar intervertebral foramen surgery, the operator can see the location of the puncture needle in real time, greatly reducing the risk of nerve and blood vessel damage. The doctors in our ward proposed that ultrasound can also be used in the field of spinal surgery. After research, it will be used for clinical treatment in 2019, avoiding the long-standing problem that the radiation generated by fluoroscopy in most orthopedic operations will cause harm to patients and operators. These major breakthroughs in orthopedic medical technology have brought profound changes to orthopedic medicine and nursing. Over the past 10 years, our nursing team has summarized a set of corresponding scientific and effective nursing systems, and worked together with the medical team to serve the vast number of patients with spine and joint diseases, giving full play to the patients we can cure. At the same time, as a medical staff who has been working in the front line for a long time, I deeply felt the pulse of the reform and development of the medical industry since the 18th National Congress, and saw the substantial improvement of the level of medical security of the people. For example, the implementation of the non local reimbursement policy allows patients outside Beijing to directly complete the settlement of medical insurance expenses when they are discharged from hospital after being treated in Beijing, without having to go back to the local for secondary reimbursement.
