Suddenly fell asleep in class? Beware of narcolepsy!

2022-03-23

With the acceleration of people's pace of life, more and more people have sleep problems. According to the 2021 white paper on exercise and sleep released by the China Sleep Research Association, more than 300 million people in China have sleep disorders, among which paroxysmal sleeping sickness is not uncommon. Due to the lack of public understanding of the disease, patients and their families often travel to neurology, cardiovascular medicine, endocrinology and other departments for treatment for many times, resulting in a high proportion of missed diagnosis and misdiagnosis, delaying the condition. The top students fell asleep in class Xiaoxue (a pseudonym), a junior high school student, has been doing well, but she seems to have changed herself in the past six months. In class, I often fall asleep suddenly and wake up suddenly after a few minutes. The students in the class argued that she was unworthy to be a study committee member. She was unwilling to go to the students to argue: "why do you say I am unworthy?" Before the words fell, she suddenly fell down. Fortunately, she was held by her classmates in time. When she panicked, Xiaoxue woke up again and asked blankly, "why did I just fall down?" Xiaoxue's parents and teachers then rushed to the scene and took her to a nearby hospital for treatment. ECG, EEG, cranial magnetic resonance and other examination results showed that there was no abnormality. Her parents took her to the sleep medical center of the First Affiliated Hospital of Jinan University. The doctor conducted a detailed medical history collection, mental examination, various physical examinations and questionnaires related to emotion and sleep. At the same time, he also completed night polysomnography (PSG) and multiple nap sleep latency test (MSLT). The examination results clearly pointed to a disease, namely narcolepsy. The rate of missed diagnosis and misdiagnosis is is high What kind of disease is narcolepsy? Professor Pan Jiyang, director of the sleep center of the hospital, told reporters that narcolepsy is mainly characterized by uncontrollable drowsiness, paroxysmal collapse, sleep paralysis, sleep hallucinations and night sleep disorders. The prevalence rate in China is estimated at 0.04%. It is the second leading cause of excessive daytime sleepiness after sleep related respiratory disorders. This disease is a lifelong sleep disorder, which can seriously affect the quality of life of patients, and even endanger their lives due to accidents. "The etiology of narcolepsy is not clear. It is generally considered to be the result of the interaction between environmental and genetic factors." Pan Jiyang pointed out that patients with narcolepsy have symptoms of excessive daytime sleepiness, which is manifested as an irresistible sleep attack that occurs suddenly when walking, eating or talking, especially when external stimuli are reduced, such as reading, watching TV, driving, attending classes and meetings. Sleep lasts for several minutes to tens of minutes, which can occur several to dozens of times a day. Most patients can be sober after a short nap, but they can't last too long. In addition, more than half of patients with narcolepsy can see limb weakness, sagging or even sudden collapse, especially when they are prone to strong emotional stimuli such as anger and laughter. During the attack, patients often have clear consciousness, no memory impairment, and can recover completely. Pan Jiyang pointed out: "due to the lack of public understanding of the disease, patients and their families often travel to neurology, cardiovascular medicine, endocrinology and other departments for treatment for many times, resulting in a high proportion of missed diagnosis and misdiagnosis, delaying the patient's condition." There are three main treatment methods Pan Jiyang said that at present, there are three main ways to treat the disease: 1. General treatment, such as regular and sufficient night sleep; Planned lunch break during the day, etc. 2. Drug treatment, such as psychostimulants (such as methylphenidate hydrochloride, modafinil), anti quenching drugs γ- Sodium hydroxybutyrate and other drugs. 3. Psychological intervention, such as understanding patients and encouraging them to adopt a positive and healthy attitude towards life. It should be reminded that if paroxysmal sleeping sickness is not diagnosed and treated in time, it will often be accompanied by problems such as anxiety, depression, social phobia and obesity, which will be life-threatening in serious cases. Therefore, we should be alert to the occurrence of this disease. See a doctor at the medical center as soon as possible and make a diagnosis and treatment as soon as possible. (outlook new era)
